Hi karenscats,

I believe the key should be in a safe in the managers office. You get the code from your journal. There is a picture below that shows the location of the key:

After taking a look through for you, it looks like the to find the scalpel:

It should be found in the hidden object scene with the baby carriage.

If you click this link here: hidden object scene picture link, it will take you to a picture of that scene, and the scalpel should be the object outlined with the number 14 next to it.
